This new 'Transition to School Pack contains lots of useful resources to make the child's transition to school as smooth as possible. It contains:

A special 'Starting School' poem to print out and share with the child and their family. - This is a lovely gift to give to families when saying goodbye. 

A specially designed 'Transition' form to help you share all the important information the child's new teacher will find useful. The form contains sections to include a photo of the child and their basic details, space to describe their characteristics of effective learning, a short check-list of the skills that are most useful and space to describe the child's development in all seven areas of learning. There is also space for any other comments you would like to make and space for comments from the child's parents and the child themselves, 

A completed sample transition form to help give you some ideas of things you can write.

A 'Sharing Information' permission form.

An information sheet with ideas parents can try at home to help get their child 'school ready'.

Activity resources to share with parents to do with their child at home.
